READ CAREFULLY BEFORE SIGNING.

In consideration of being allowed to participate in any way in the activities, events, and programs of JJ Ultimate Sports Training LLC, doing business as Run Speed Performance ("RSP"), I, the undersigned, on behalf of myself, any minor listed above, and each of our respective personal representatives, heirs, next-of-kin, and assigns, acknowledge and agree as follows:

1. ACKNOWLEDGMENT OF RISK. I understand that participation in athletic training, speed work, strength training, agility drills, conditioning exercises, combines, camps, and related activities involves inherent risks of physical injury including but not limited to: muscle strains, sprains, fractures, dislocations, concussions, heat-related illness, cardiac events, and in rare cases, permanent disability or death. These risks exist whether training occurs at RSP's facility, outdoor locations, public parks, school tracks, partner facilities, or any other venue.

2. ASSUMPTION OF RISK. I knowingly, voluntarily, and freely assume all such risks, both known and unknown, related to my or my child's participation in RSP activities, even if arising from the negligence of RSP, its owner, coaches, employees, certified trainers, or agents.

3. RELEASE AND WAIVER. I hereby release, indemnify, and hold harmless JJ Ultimate Sports Training LLC (DBA Run Speed Performance), its owner, coaches, employees, certified trainers, independent contractors, affiliates, and the owners and lessors of premises used by RSP to conduct its activities, from any and all claims, actions, causes of action, liabilities, demands, losses, damages, suits, and expenses (including reasonable attorney fees) arising out of my or my child's participation in RSP activities, use of RSP facilities or equipment, or presence at any RSP training location.

4. FITNESS REPRESENTATION. I represent that the athlete is physically fit to participate in RSP training activities and has no medical conditions that would prevent safe participation, or that all such conditions have been disclosed in Section 4 of this form. I agree to notify RSP of any changes in the athlete's health status.

5. MEDICAL AUTHORIZATION. In the event of an injury or medical emergency, I authorize RSP coaches and staff to seek emergency medical treatment for the athlete. I understand that RSP does not provide medical insurance for participants and that I am responsible for all medical costs incurred.

6. OFF-SITE TRAINING. I understand that RSP conducts training at various locations beyond its primary facility, including outdoor parks, school tracks, indoor sports complexes, and event venues. This waiver applies to all locations where RSP operates. I understand that RSP cannot guarantee the conditions of third-party facilities.

7. WEATHER CONDITIONS. I understand that outdoor training may expose the athlete to weather-related risks including heat, cold, rain, lightning, and uneven terrain. RSP coaches may modify or cancel sessions due to weather at their discretion.

8. TRANSPORTATION. I am responsible for transportation of the athlete to and from all training locations. RSP is not responsible for the athlete before check-in or after check-out.

9. GOVERNING LAW. This agreement shall be governed by the laws of the State of Texas. Any disputes shall be resolved in the courts of Dallas County, Texas.

10. SEVERABILITY. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

I grant JJ Ultimate Sports Training LLC (DBA Run Speed Performance) permission to photograph and record video of the athlete during training sessions, camps, combines, and events. This media may be used for coaching analysis, RSP website and social media, marketing materials, and promotional content. RSP will not publish a minor's full name alongside their image without separate parental consent. I may revoke this consent at any time by written notice to info@runspeedperform.com.
